New Patient Appointment Information
You have submitted the appointment booking request. Within 3 business days, you will receive an appointment confirmation from our electronic health record, Practice Fusion (or Patient Fusion).
To prepare for your upcoming visit, we request you complete the following items.
Complete items 1-3 NOW, and items 4-5 upon receipt of your reminder email, one week prior to your appointment.

- Our providers require your records to be on file prior to your visit if you are transferring your care from another provider or if we need to coordinate your care with other specialty providers. If there are no prior records, you do not need to submit the form.
- Details regarding our Prices are available on our website.
- The appointment confirmation and link to the intake form will arrive in an email reminder. Click Confirm. Complete the form in its entirety. Click submit.
- Our providers kindly request this intake form be completed 2 days prior to your visit, or your visit may need to be rescheduled.
- Tell us everything we need to know about why you're coming to see us at Integrative Family Medicine. The transfer of your records and the completion of the online intake form allow your provider to best utilize your time with you. Give us as much information as you can!
- The reminder will give you the date, time, and address of your upcoming appointment. Please verify the address of the appointment location, as we have 3 different offices.
- Upon completion of the online intake form, it will prompt you to create a patient account.
- The patient account will allow you to link to your chart and view any test results, change your personal information, and communicate directly with your provider.
- The patient portal (Patient Fusion) is our preferred & primary method of communication with our patients, as it allows for the fastest response time by our team members.
Reschedule or Cancellation Policy
If you must cancel your appointment or reschedule, please do so 3 business days prior to your appointment, otherwise our Reschedule/No Show Fee of $99 may be charged if the appointment is changed less than 48 hours/2 business days before the appointment.
