For services prior to March 1, 2023: If you received a statement with a patient balance and you would like to pay online, go to www.myproviderlink.com. You may use the guest pay option to enter the information from your statement to pay or you may set up an account to securely keep your information. Contact your pharmacy to initiate the refill request and the pharmacy will send us the request electronically. If you need a refill on a controlled medication and have an agreement with your provider, please use your patient portal to contact your provider and follow your medication refill process that was agreed upon with your provider. what types of new patients are you accepting?
For newborns, please call our office at 515-218-1399, option 1, or submit a newborn appointment request 24/7 on our website. Our online form allows you to submit at your convenience once the little one arrives. Our office will contact you within 1-2 days to schedule the appointment.
